When submitting a Drivers application please include all attachments (A, B & C)

You must PRINT out all the attachments and either fax (701.575.4160) or mail (PO Box 458, Belfield, ND 58622) it in.

B – FCRA Authorization Release (required)

C - Prospective Employee Questionaire (required)

Note: If you are filling out an application–please complete it entirely before submitting. DOT requires Driver Applications to be in applicant’s own handwriting, therefore applications are not in fillable form.
